thats a neat idea, Tito. I would really like to see the screening questions (sounds like they include more than just DV). figuring out how the different computer systems can "talk" to each other is always a challenge, but not insurmountable. We have a screening questionnaire that is offered to the patient when they are being roomed so that the info goes into the electronic medical record, but that has its challenges- the questions are being asked by a medical assistant or clinician, which is different from an anonymous kiosk that provides information right then. thanks for looking for the opportunities for connecting these ideas.

Regarding the first component of your delivery model, the screening and referral system, you mention an electronic medical record; I wonder if this particular method might benefit from referring to the innovation entry titled: "Emergency Department Computer Kiosk" (http://www.changemakers.net/en-us/node/148). This program appears to have developed an intriguing touch screen kiosk module for screening patients in emergency and waiting rooms. Perhaps a useful connection could emerge?
